Middletown, NY – November 23, 2009- Crystal Run Healthcare was recently granted Level 3 recognition by the National Committee for Quality Assurance (NCQA) through the Physician Practice Connections-Patient Centered Medical Home (PPC-PCMH) program for using information systems to measure practice-wide clinical quality and improve clinical outcomes at point of care.
Recognition was granted to the Practice’s 155 and 300 Crystal Run Road offices in Middletown, as well as its Rock Hill, Harriman, Goshen and Monroe offices. PPC-PCMH recognizes practices that use information systematically to improve the quality of patient care. Practices that meet the program standards monitor for patients who need care, measure quality across their entire practice and ensure that abnormal test results receive follow-up, thus improving medical errors and reducing errors.

O’Kane. “Physician Practice Connections-Patient Centered Medical Home identifies those doctors who make the most out of clinical information they have on hand to make better decisions and provide better follow-up. Patients notice and appreciate the improvements in care. The physicians at Crystal Run Healthcare are to be commended for their achievement in earning Recognition.”
In 2006, Crystal Run first received recognition from the NCQA through the Physician Practice Connections (PPC) program, the only national program to recognize physicians who organize their practices around quality care using electronic systems to assist them.
