In addition to our state-of-the-art medical technology, Crystal Run Healthcare was among the first few medical practices in the country to adopt the use of electronic health records (EHR) — a groundbreaking tool that lets our providers, nurses, technologists and support staff share a patient’s information instantly to minimize error, initiate care sooner and maximize communication between and among everyone on the healthcare team.

Choosing a Primary Care Provider

Your primary care physician (PCP) is responsible for managing and coordinating all your healthcare needs, and we want to help you make the ideal choice.

Your PCP will help you with prevention and wellness strategies, and will coordinate all appropriate health screening tests, such as mammograms, pap smears, immunizations and colonoscopies.

But that’s not all. Your PCP will also coordinate any care you may need from our renowned staff of Crystal Run specialists. The quality of care we provide locally has been recognized nationally.

Pathology Slides/Block Consent and Release Form

To have your Pathology Slides sent to another facility or provider outside of Crystal Run Healthcare, please fill complete and return this form.

Dr. Hal Teitelbaum is the Founder, Managing Partner and Chief Executive Officer of Crystal Run Healthcare—a premier multi-specialty group medical practice. Dr. Teitelbaum completed his training in Internal Medicine at New York Hospital-Cornell Medical Center, and in Medical Oncology and Hematology at Memorial Sloan-Kettering Cancer Center. After beginning his career in full-time academic practice, Dr. Teitelbaum established a solo practice in Orange County, New York, in 1982. He established Crystal Run Healthcare in 1996. Dr. Teitelbaum is a 1998 Honors graduate of Columbia Business School, where he earned his MBA. He earned his Juris Doctor degree from Columbia Law School, Class of 2012, where he was a Harlan Fiske Stone Scholar.
