Middletown, N.Y. – February 26, 2013 – The National Committee for Quality Assurance (NCQA) has announced that Crystal Run Healthcare has once again received Recognition as a Patient-Centered Medical Home (PCMH). The NCQA has designated Crystal Run Healthcare as a Level 3 Patient-Centered Medical Home, the highest level achievable for a PCMH. The same level of recognition was reached by Crystal Run in 2010; this latest recognition is again valid for three years. This recognition by the NCQA demonstrates that Crystal Run has met the program’s key elements embodying characteristics of the medical home including:
  • Written standards for patient access and continuity of care
  • Use of patient feedback materials
  • Appropriate use of charting tools to track patients and organize clinical information
  • Responsive care management techniques with an emphasis on preventative care
  • Adaptation to patient’s cultural and linguistic needs
  • Use of information technology for prescriptions, test and referral tracking and coordination with other health care providers
  • Use of evidence-based guidelines to treat chronic conditions
  • Measurement and reporting of clinical and service performance
“We are once again honored to receive Recognition by the NCQA for our hard work as a Patient-Centered Medical Home. Maintaining our Level 3 status is paramount to our commitment to Value Based Care for our patients. It is this commitment that drives us as an organization as we continue to improve patient care and outcomes,” said Hal Teitelbaum, MD, JD, MBA, Managing Partner and CEO of Crystal Run Healthcare.
The NCQA’s Patient-Centered Medical Home ™2011 (PCMH 2011) program recognizes health care organizations for the use of evidence-based, patient-centered processes that focus on highly coordinated care and long-term participative relationships. The standards are aligned with the joint principles of the PCMH established with the American College of Physicians, the American Academy of Family Physicians, the American Academy of Pediatrics and the American Osteopathic Association. The patient-centered medical home is a model of care that emphasizes care coordination and communication with the patient. A PCMH such as Crystal Run Healthcare identifies practices that promote partnerships between individual patients and their personal health care providers, instead of treating patient care as the sum of several episodic office visits.
“The Patient-Centered Medical Home raises the bar in defining high-quality care by emphasizing access, health information technology and partnerships between clinicians and patients,” said NCQA President Margaret E. O’Kane. “PCMH Recognition shows that Crystal Run Healthcare has the tools, systems and resources to provide their patients with the right care at the right time.”
