At Crystal Run Healthcare, we strive to eliminate every barrier between you and good health. That’s why we accept a long list of insurance plans.

IMPORTANT NOTES-
As of January 31, 2021, we will no longer be accepting any Fidelis Health Insurance plans.
Members can change to a different plan by calling the New York State Medicaid Choice, New York State's managed care enrollment program, at 1-800-505-5678.
As of January 31, 2021, we will no longer participate in Workers’ Compensation.
