Crystal Run Healthcare Pediatrics Art Contest Submission Guidelines
Each entry must meet the following guidelines:
• Original drawings/paintings (hard copy) must be submitted in a format that can be hung.
• Any medium is accepted, ie; pencil, crayon, paint, watercolor, charcoal, etc. (no photography please)
Crystal Run Healthcare Art Contest Official Rules
By entering the Crystal Run Healthcare Pediatrics Art Contest you agree to be bound by these Official Rules.
1. ELIGIBILITY.
Open to children (any person 18 years of age or younger).
2. CONTEST DESCRIPTION.
The Contest is for children (any person 18 years of age or younger) and intended to promote creativity. The object is to submit one piece of art to be judged. Winning participants will have their art displayed at Crystal Run Healthcare’s 100 Crystal Run Road Pediatric location in Middletown, NY and posted on social media and on our website.
Entry Period
The Contest is open from April 15th - May 15th, 2021 (Entry Period). All entries must be submitted prior to the close of the Entry Period. Any entries submitted after the deadline of May 15, 2021 will not be accepted.
3. HOW TO ENTER.
Entrants must fill out the submission form below and print. Participants will be asked to provide their first and last name, title of artwork being submitted, a brief description of artwork and contact info/email address.

Completed registration forms must accompany each entry. Attach one (1) copy to the back of the entry being submitted. Entries can be dropped off to any Crystal Run Pediatrics reception desk located at:

Entry criteria
All entries submitted must be original and must have been created and owned exclusively by the entrant submitting the creation. Participants acknowledge and agree that winning artwork will be hung up at the 100 Crystal Run Road, Middletown, NY, Pediatrics department and viewed by the public as well as posted on social media and our website, www.CrystalRunHealthcare.com.
4. JUDGING:
A panel of judges will review eligible entries and select winners based upon the following criteria:
• Overall Impact (inspiring, persuasive)
• Creativity
• Originality
Judges will announce the winners, on or about June 1, 2021. Winners will be notified via phone and/or email.
5. PRIZES:
Winning participants will have their art displayed at Crystal Run Healthcare’s 100 Crystal Run Road Pediatric location in Middletown as well as posted on social media and on the Crystal Run Healthcare website.
6. CONDITIONS OF PARTICIPATION.
This Contest is void where prohibited. Acceptance of a prize constitutes permission for Crystal Run Healthcare to use that winner's name, likeness, picture and entry without additional compensation unless and where prohibited by law. By entering this Contest you agree to and hereby release and hold harmless Crystal Run Healthcare, its employees and agents from any and all liability or any injuries, loss or damage of any kind arising from or in connection with your participation in this Contest, or any prize. If you do not want to grant the foregoing release, do not enter the Contest. Your entry must be your own original work. By submitting your entry in the Contest, you represent and warrant that it is your sole original work, and that you have obtained any and all consents, approvals or licenses required for you to submit your Entry and comply with all of these Official Rules, including obtaining the written consent of all 3rd parties, where applicable. You waive any claim of infringement (including without limitation copyright, trademark, patent, trade name, trade secret, etc.) against Crystal Run Healthcare or any employee or agent of Crystal Run Healthcare based upon access to or use of your entry.
7. LIMITATION ON LIABILITY.
Crystal Run Healthcare is not responsible for any incorrect or inaccurate information, whether caused by website users or by any of the equipment or programming associated with or utilized in the Contest, or by any technical or human error which may occur in the processing of entries. Crystal Run Healthcare assumes no responsibility for any error, omission, interruption, deletion, defect, delay in operation or transmission, communications line failure, theft or destruction or unauthorized access to, or alteration of, entries. Neither Crystal Run Healthcare, nor any of its employees or agents, is responsible for any problems or technical malfunction of any telephone network or lines, computer on-line-systems, servers or providers, computer equipment, software, failure of email on account of technical problems or traffic congestion on the Internet or at any website or combination thereof, including injury or damage to participants or to any other person's computer related to or resulting from participating or downloading materials in this Contest. Crystal Run Healthcare reserves the right at their sole discretion to cancel, terminate, modify or suspend the Contest for any reason.
8. OTHER.
Illegible, unintelligible, inappropriate or incomplete entries will be disqualified. Without limiting the foregoing, Crystal Run Healthcare may disqualify any entrants from the Contest who deliberately damage, tamper with or undermine the legitimate operation of the Contest.
