Facility Obtains Highest Achievable Level from the National Committee for Quality Assurance (NCQA)

West Nyack, N.Y. – November 20, 2017 – Crystal Run Healthcare’s pediatric office in Haverstraw was recently granted Level 3 Patient-Centered Medical Home (PCMH) recognition by the National Committee for Quality Assurance (NCQA) – the highest achievable level of recognition that a patient-centered medical home can receive.

The NCQA Patient-Centered Medical Home is a model of primary care that combines teamwork and information technology to improve care, improve patients’ experience of care, and reduce costs. Medical homes foster ongoing partnerships between patients and their personal clinicians, instead of approaching care as the sum of episodic office visits. In receiving Level 3 Patient-Centered Medical Home recognition, the NCQA has verified that Crystal Run has made a commitment to providing quality improvement within its practice and employs a patient-centered approach to care, resulting in happier and healthier patients.

“Crystal Run Healthcare’s Pediatrics office in Haverstraw has been awarded recognition by the National Committee for Quality Assurance Patient-Centered Medical Home Program (PCMH). NCQA Patient-Centered Medical Home Recognition raises the bar in defining high-quality care by emphasizing access, health information technology and coordinated care focused on patients,” said NCQA President Margaret E. O’Kane. “Recognition shows that this office has the tools, systems and resources to provide its patients with the right care, at the right time.”

“I’m proud to say that our Pediatrics office in Haverstraw has earned the highest level of Patient Centered Medical Home recognition by NCQA. The Providers and Staff exemplify the uppermost quality standards for patient care which are the benchmark of our Practice and show a proven commitment to the Mission of Crystal Run Healthcare.  Congratulations on this achievement,” said Roseanne Baker, LPN, Care Optimization Manager at Crystal Run Healthcare.

The NCQA’s Patient-Centered Medical Home Recognition Program is the most broadly endorsed PCMH evaluation program in the country, with more than 12,000 practices (with more than 60,000 clinicians) recognized.
